I've had one serious quest bug and lots of things to do with Druid Wild Shapes haven't worked properly (seems to be fixed with the patch). A couple of instances of people being able to see me sneaking through walls.
Lots of smaller glitches sprinkled on top, but nothing else that's been a major problem. Maybe I've been lucky, but in a game this big it doesn't seem bad. I'm noticing a general lack of polish in Act 3 though, it seems a bit rough around the edges in general, but I still have plenty to go so I can't judge it yet.

I'm only in 2 and haven't noticed anything worse than act 1 yet In terms of bugs. Had to reload save a couple of times but that's it. It's pretty astonishing that a game this big works as well as it does.

The only bugs I've noticed is people 'sliding' about instead of walking for a few moments. I didn't pay enough attention to even be sure when it happens. Agree travelling to a new location, saving, loading, exciting dialogue. No clue.
Earlier tonight I saw a trap floating in mid air, because the bridge it was on collapsed.
That's it really.

Late-ish Act 1 spoilers: After defending Emerald Grove, I'm struggling to locate Halsin. I'd released him from the Goblin Camp, and I did venture back that way, though every goblin is now hostile. From the Blighted Village, I've spied a way in down an embankment to a stone bridge over the river, though the jump will cost 10HP: I'm wondering if it's worth it? I would like to visit the Underdark entrance I discovered there.
Edit: Been fun reading other strategies on Reddit - so many things I never thought to do. Might have made life hard for myself by not activating a waypoint

He followed me around for a bit as I murdered the last goblin leader, then he said bye bye and went back to the druid grove, where I found him downstairs close to where the idol is. I've no idea how it plays out if you do things differently of course. If you haven't been to the Underdark I'd highly recommend it. Lots to explore, quests to do, some NPCs to meet that can turn up in later acts and some darn sweet gear to get that I'm still using in Act 3.

Currently on my way to the moonrise towers, must be pretty close. Was just in the creepiest part of the game I'd encountered so far - throwback vibes to many a horror game setting, was sweet. Managed to convince a room full of people, including a boss, to kill themselves which was nice.
And then a gith beeper went off and a bunch of gith ambushed me. Annoying but whatever, they dropped some pink gith super gloves which are a bit fancy.

Then he joined me in rampaging through it and killing the 2 leaders I had left to kill. After that, he was back at the Grove, down where they were doing that ritual. For your other conundrum. No spell in anyone's spellbook that can help you out? Or a weapon with a spell attached, a potion to drink, a scroll to use? Fly, featherfall, misty step and that other spell that give you triple jump distance could all be useful.

Oh, this game cheats — and in plain sight. Goblins are all on the bottom floor. I wondered how they were reaching me so quickly, then watched one run into the wall and magically appear on my level. Fuck off.

Resting really isn't a limitation in the game, as long as you occasionally pick up extra food and drink, and there are very few places where you can't rest. You don't even need food for a short rest. You get 2 of those (more if you have a Bard), and you get half your HP back. It's like a free heal!

Anyway, I'm still in act 1. I haven't had many bugs aside from some floating glassware. Hopefully by the time I get to act 3, most of the more serious ones will have been patched out.

I love the idea of the Dark Passenger/Urge. I feel like most players in these games are cowardly goody-goodys who hammer Quick Load if they so much as hurt anyone's feelings, let alone make evil choices.
But if the game randomly chooses murder targets for you, out of your control, maybe it will make the do-gooders feel less bad about it. And will open them up to the joys of absolute (virtual) bastardry.

I was planning full evil but I would have lost a party member that I wanted to use on this run. Full evil is next, help the goblins kill the druids and then kill all the goblins is the act 1 plan.

Well, Halsin has gone AWOL. Cleared out the Goblin Camp, so all Big Bad Goblins are all dead, been back to Emerald Grove, been back to camp and nada. I've been scared off the Mountain Pass, and I've still got 1000XP to go to hit level 5. I don't mind being a completionist, but I'd like to move the plot forwards.

If you don't want to miss anything major I'd recommend doing the mountain pass first (where Lae'zel's quest points you) as there's some big story for her, Gale and the main plot in that area, and at least one awesome item to find. I have no idea what happens if you skip that bit entirely but it seems like a lot to miss out on.
Then when it prompts you to move on (after meeting Gale's friend) you can either go from there or go back through the Underdark.

You meet the friend regardless of route. It's cleverly designed so you can go one way and leave the other for a second playthrough. You can do both but there isn't much need other than the odd item. Of course a certain amount depends on who you take in your party as to which way you should choose though.

Ah that's neat. I assume you can go back and do the crèche before you do certain things late in Act 2, but it's some pretty big stuff if you skip it entirely (not just for Lae'zel). Then again, maybe there's something else that moves the whole Voss, Vlakith, Guardian thing on if you don't go there at all. That legendary weapon though.

You don't want to be wandering into some of those act 3 bosses underlevelled and underprepared. Some of them are bloody tough, I had to give up on one and take another path as he was smashing me. The game did warn me it was a bad idea, I just didnt believe it haha.

It's actually insane how much they've crammed into Act 3. I still have half my companion quests to tie up, a bunch of side quests, and there are areas of the Lower City map I haven't gone to yet. The map isn't even that much bigger than previous acts, it's just insanely dense with stuff.
My druid is... bad. Early-mid game it felt pretty strong, but towards the end game it becomes a real liability that you don't scale with gear and she's falling way behind the other party members in both damage and utility. It's not really a problem as combat isn't that hard and the rest of the party is carrying, but it doesn't feel good.
Halfway through Act 3 I realised that the Longstrider spell is a ritual spell out of combat (doesn't use a spell slot) and lasts until the next long rest, so my entire party could have had extra movement through the entire game if I'd only read the spell descriptions properly. Oops.
