Crafting, unless it’s really, really well implemented.
Base building.
Constant grunting in Square Enix voiceovers.
When a game has collectible junk that you’re supposed to sell but is indistinguishable from items that prove useful later on.
Too much weapon and armour “variety”. E.g. contrast MGS1 to MGS4, or FFVII to FFXIII.
When one route is obviously the exit and one route obviously contains a treasure yet you have no idea which and you’re afraid to commit to going the wrong way.

The way no one admits that pre-rendered backgrounds, or at least a fixed camera, actually allows for a great more visibility than a third person camera

I’m not a big fan of overly abstract puzzles. Ones you would never work out without a guide.
And repeated levels / bosses. Don’t mind it if there’s a point to it or if it’s significantly different, but hate it when it’s just padding out the game’s length.

A fair few mentioned here like instafail forced stealth missions. However one of my biggest pet peeves are when the game decides to hobble you about 2/3 of the way through by removing all your abilities that you spent the whole game earning in an effort to pad out the play time. Oh and games that lock out basic abilities that really should be standard that you have to unlock.

Reminds me of Doom 2016 having loads of secrets but not allowing you to backtrack so you have to stop and look as you go along, which kind of messes with the pacing a bit. Glad they addressed it in Eternal by unlocking fast travel at the end You can revisit completed stages in 2016's campaign mode though

But related to that, being actually unable to backtrack for stuff in any way. I hated this e.g. in ME2, where the only way to earn money was finding some in a container, and getting some for completing a stage. Since the amount of money is finite and much too low to buy all I want, I spend far too much time in every stage searching every last corner because I might miss a few credits otherwise. Never been an issue in ME1 because you can, well, sell loot.

Bosses with hidden 2nd or third phases - for instance, as much as I loved Slay the Spire, the Awakened One boss in Act 3 infuriated me.
Caveat: sometimes this mechanic can work well e.g. the Guardian Ape/Headless Ape fight in Sekiro. Mostly, though, it annoys the hell out of me.

Trippy/dream sequences or ones where you have to relive past shit just to give back story that completely remove all agency I had as a player.
Hate them, just feel like you aimlessly wander in different directions hoping to trigger the next step usually while your vision, speed, health or some other stat is adjusted off the scale.
Far cry is usually a fucker for this but it happened in dying light 2, Alan wake, control and many others.
I feel like borderlands took this shitshow and went whole hog with it .. fight for your life. Its just dogshit.

I don't like having completion of a game, or the "best ending", gated behind doing meaningless side stuff. Arkham Knight was the worst for it, but there's probably loads of other examples. If you want me to do "challenges" and collect dodads, fine. But it should be 100% optional.
I also don't like the "best ending" being gated behind impossible platform hell challenges and/or superbosses. I'm looking at you, Hollow Knight. If you want to have that kind of stuff, put it in as a post-game bragging rights reward. But I want to be able to finish the game I paid for, even if my skills aren't always up to it. (I have a small exception for roguelikes, where coming back from failure is a valuable experience for me)
On that note, one thing I really support, and wish more games would do, is modifiable difficulty. There's a bunch of games now that have really detailed accessibility and "assist mode" options. Celeste is a good example, though it's not really my kind of game. I even support it for roguelikes; Darkest Dungeon and Invisible Inc. had great options in this regard.

Romances
Sexy funtime cutscenes, romance options, bin them all.
First they're cringe as all fuck; knock-off Twighlight fanfic reads as Shakespeare by comparison. Second, the gamification is icky - your intended has no agency, knickers drop once requisite number of gifts have been delivered or enough convo options with hearts have been selected. Third: frequently the player is the boss of the person they're boinking. You're a walking talking MeToo hashtag and the Space Army HR department wants a word, Commander Shepherd.
But mostly it's because of the epic eye-roll I get from my other half when she glances over to the ray traced elf tits on my screen and asks "are you banging the pretty elf lady?". "It's for a quest!" I protest, as Geralt's buttocks grind away.

I think a better system than 'you are overencumbered and you cannot run' could be to make you drop something before you pick it up maybe though. Yeah. Just have an item limit rather than forced tiptoeing. But you can put that limit on yourself by dropping the thing you just picked up, or a different thing that you need slightly less. If I’m in the Dwarven Halls place under Skyrim and I find a heavy weapon I want more than anything else in my inventory, and there’s an item limit rather than encumbrance, all I’d do is drop other stuff to pick it up anyway. With encumbrance I can pop a potion to temporarily boost the limit and fast travel. Or crawl to the nearest safe place. Or, and here’s the kicker, do the exact same thing I’d have done if there was an item limit and drop some other stuff. The absolute last thing to do is go “oh I’ve hit the limit, I’ll just leave this cool thing here because I can’t pick it up now”

Win the battle, lose in the cut-scene. Trails of Cold Steel, I'm looking at you. Hate this. Also the slight variation, RPG boss battles that appear difficult but winnable where you end up using tons of healing items and magic, only for the boss to one hit kill your entire party.

Boss fights when you hit the enemy and realise it's going to take about 20 minutes to kill it. The fight isnt hard or challenging just a massive tedious slog of plinking away. Boss fights in general.

Here's a few:
Time limits - I like to be meticulous and take my time, whenever a game forces a time limit on me it just stresses me out and makes me feel like I can't enjoy the game properly
Endlessly respawning enemies
NPCs you have to protect - thinking mainly of TRPGs like Fire Emblem and Tactics Ogre here. In either case, said NPCs will invariably march headlong into battle and get themselves killed as quickly as possible turning the mission into a massive pain the arse that wouldn't be the case if they just did the sensible thing and let you either control them or have the AI move them to the back of your group where they're safe
Dark Souls mechanics in anything that's not Dark Souls - stamina bars, overly punitive deaths, etc
Roguelikes
Stat tracking in single player games - I don't need a record of how many times I've died/how shit I am
Unskippable start up splash screens - I've been playing games for decades and I still don't know what Criware is, nor do I give a shit
Vignetting at low health - putting shit all over the screen when I'm nearly dead doesn't help me stay alive, it means I'm more likely to die because I can't see what I'm doing
Crafting - why have just one item when you can have 5 items to make the one item
Video games are shit aren't they
God yes time limits. I spend all day at work with too much to do and too little time to do it in. Don't put that into my leisure time.

Bosses who summon 'helpers' - if your that of a 'boss' do it on your own FFS. I hate boss battles anyway - tedious and archaic mechanic.

Make the "best" ending the default ending because sometimes I just don't want to collect a bunch of random shit just to get what is the decided canon end come a sequel.

I think Fight For Your Life is a great mechanic but dream sequences usually are a bit shit
