I have two.
First, a commercial fishing industry simulator. You manage a fishing fleet and docks, you design your vessels and choose type and location of fishing. Also fish processing plants, distribution, marketing etc. Potential for a fish and chip shop DLC.
Second, a Carry On-themed fighting game. Seriously, so much scope for characters, arenas, special moves. Who doesn't want to see Sid James vs Hattie Jacques?
I doubt either of these would be commercially successful.

I honestly think a commercial fishing simulator would find an audience, it's basically a business management game and those can do reasonably well.
The Carry On fighter, less so...
I did actually start to write a pitch for a game concept that I frequently ponder when Im lost in reverie, but it ended up being *several paragraphs*, as I went into detail of the systems that I imagine the game having, and realised it was just nonsense.

I should say that Carry On Punching is a fully 3D arena fighter a-la Powerstone. Babs Windsor vs Kenneth Williams in the dining room from Carry On Up the Khyber.

More ideas for certain types of games rather than full-blown game ideas.
I would like to see a game like Baldur's Gate 2 except that the potential party characters would go on doing their own thing if they were not admitted to the group. Not necessarily something very detailed, but just having a few fixed events - perhaps with the possibility for the player to influence outcomes at certain points - that would only happen if characters were not included in the group or if the player ended up not doing their character quest.
The second idea would be an ARPG, but with the unique items being a big deal in terms of lore. So they would be hand-placed with traders, bosses and in certain areas. There would be information in books, with scholars, traders, adventurers and such. The items would always exist in the world though, no matter if the player has any info on them, so it would be possible to just stumble across them.
And third: An ARPG or RPG where only a few character classes would be available (though perhaps with specializations) that would experience the game very differently based on their background. Having access to certain areas, certain groups being hostile, important dialogue options and/or quests only showing up for certain characters etc.
All of these could theoretically be combined in one game, but that would likely be too much and perhaps not easy to reconcile.

Carry On Grinding
It's an RPG whose name is also an obvious innuendo.
You play Mr. Dickens who is on a quest to launch his own apple-based alcoholic drinks empire. On the way you'll meet a camp accountant, a delivery driver with a cackle, a prissy secretary who is offended by everything and a down-on-his-luck orchard owner.
Your dream of Dickens' Cider will be fulfilled.
At some point a young Barbara Windsor gets her baps out.

A rap battle game with full speech to text.
An AR skateboard game using GPS RTK to determine the player's location. Rolling fast in unusual locations like school floors, libraries and museums (police stations?) gives extra points.
GTA but with boats in a Chinese floating city.
An eco-terrorist game where you have to sabotage new construction projects and industrial plants and so on. Gameplay like X-Com.
The Sims, but you don't control people. You are actually Satan and have to try to corrupt them and make them swear their souls to you and try to form a satanic cult.
A 4x space game, but you play the universe and have to get rid of the space faring civilizations by strategically combining weather patterns, planetary catastrophes and intergalactic shenanigans.

Start off catching lobsters from a punt close to shore, then go up the tech tree - Lobster -> Crab -> Whitefish Inshore -> Whitefish Offshore -> Tank Boat -> Factory Ship
Go from being out on deck to managing modern instruments - the wheelhouse of a modern boat these days is like the cockpit in Elite Dangerous
Get your business torpedoed by EU Quotas/Brexit Fuckery and have to go fish the West Africa DLC during the summer.

Pretty certain there is a commercial Fishing Simulator. If it featured Dead Bod and St Andrew's Dock circa '75 I'd be in like Flynn. Could feature a DLC featuring additional unknown crew member'from a government dept' monitoring radio in waters off Murmansk. Was just going to post that, well actually the new one Fishing: North Atlantic. I only found it yesterday. Going to download it when the kids get off of the tv.
I sunk a lot of time into Fishing Sim World, it was great but Dovetail then went full Bass and stopped supporting it.and haven't tried that one yet. FSW was the best fishing game I've found but so much more can be done in a game. Different locations, salt water species and all can be added.

A racing game, but you can't control the speed, only the steering wheel. It would be more of a puzzle game than a racing game. Maybe it could even be turn based.

I believe Microsoft owns the rights to Shadowrun video games? To the extent that Harebrained Schemes, which is led by the creator of the tabletop RPG, had to license it to make the Returns trilogy. So now that Microsoft also owns all the big western RPG studios*, I’d like to see a full AAA open world Shadowrun set in a cyberpunk Seattle. I guess 2077, but with magic and dragons and elves and things. Have Obsidian make it I guess, since they’re good at “what that company did, but buggier.” * apart from the two studios that matter most, Bioware and CD Projekt Red

you need to manage and motivate staff to complete work on ever shorter deadlines while the clients expect ever increasing quality. Do you go for a big team of shit cheap people or a small quality team that could get headhunted at any second? Do you stick with accurate quotes or compete on cost to attract work? How do you solve the HR fallout from a Christmas Party?

You have to hack a system. You do so my clacking away at a keyboard like in the films. Random keys progress the hack while you keep an eye on the police’s attempts to track you, using various power ups to lead them astray.
Finish every level with a satisfying punch of the return key and shouting I AM INVINCIBLE!!!

Crazy 90s words and tubes swirling as you do it too. The more 90s special effects, the closer you are to be the cool hacker dude who listens to Prodigy and bags the unbelievably hot female member of the crew.

Some sort of game where you're a baby exploring house and neighbourhood. You analyse object properties by taking them in the mouth and smashing them on the ground, and stealth works by covering your eyes with your hands
