There's more and more research into the likely link between gut bacteria and other issues, such as depression/anxiety over the last ten years. It's a long shot but have you tried any strong probiotics? Worth giving a go along side the therapy, plus maybe minor changes to your diet to encourage 'good bacteria'.

My wife has similar episodes - not to the point of agoraphobia, but if there's a change to her routine, a big event or night out that we've been looking forward to, she'll suffer from trapped wind and invariably we'll have to cut things short. She has IBS and some anxiety, so can absolutely see that they are linked. It's less common now but that might just be because we never go out!

We’re getting our puppy today. Had to leave at 9:15. Did everything I thought would help, ate light yesterday and nothing today, had no caffeine since a single cup of coffee 24 hours ago, got up at 6 this morning so I could OD on Imodium with plenty of time for it to kick in. Gets time to leave and… all the shit. Fuck knows where it’s coming from, I definitely expel more than I intake, I should be a rake by now.
So she’s gone to pick him up without me. Meant to be the happiest day of our year and I’m not there because my brain is a cunt. Might have to dip into savings and check myself into some fancy hospital to fast-track recovery. Anyone here done that?

Baby steps, innit? Your anxiety wasn't going to go away because you were going to get a puppy.
New puppies can only be taken for very short walks (like minutes) which increases gradually with time. Maybe that's what you need? Take the pup for walks every day, minutes at a time, working up something over a matter of months. Like taking her to the local or the park or something.
I'd try CBT again. What you're describing sound like a case history in CBT class. I know you've done it before but I think it's very operator dependent. Not all clinical psychologists are equal and there's a lot of subtlety to the formulation they arrive at. Maybe you just need a different one.
And it probably goes without saying that if the shits are that bad, make sure you get that checked out as well. It sounds like just anxiety but we're not spring chickens anymore.
Make sure you talk to someone properly and not some random gamer on the internet. There's lots of options before spending money on extreme stuff.

Isn't planning how you eat around it only going to increase your anxiety? Sounds like you're thinking about it constantly. I don't think you can control your body like that
I'd focus on small trips out for the sake of it rather than big events

According to CBT, you should try just going to the door first. Then later try and open it. Another day sit outside. When you're happy with that, sit in the car. Later turn it on or really around your garden.
If it helps, I'm taking my cats to the vets in 15 minutes. My anxiety has got to the point I'm struggling for breath. So, maybe going for a puppy is pushing it for a first challenge.
One of my cats is a really poor passenger as well. Going to be like a feline remake of the Exorcist. I'm really not looking forwards to it.

I don't think anyone can really give good advice as it's a personal thing and a therapist would be in the best position to understand how your anxiety is affecting you/triggered and work out a plan on how to address it
Putting so much pressure on yourself and feeling terrible when you 'fail' isn't helping though, neither is not eating or constantly planning for when you're going out
Are you able to go for a short walk in the street? The worst thing that can happen is you have to nip home quicky. Not a disaster

You can push them back in for one thing.
Constipation or trying to hard are a cause and it's recommended that you drink a lot of water to help avoid them. Also avoid alcohol and caffeine and exercise more. Stress on its own isn't a cause, but it can lead to actions or inactions that do cause them.

Awaiting a scan on my shoulder. Not sure what's happened but it's been fucked for months. Only seems to hurt if I stretch my arm out to the side and any other similar movements like putting a coat on. Definitely don't recall any trauma to it, so I'm baffled as to what I've done. It's been like this for months and Naproxen hasn't done anything to help.
