Games that just have excessive junk laying around, with no use other than minor currency trade in but it just litters your inventory being annoying. It then also makes the "Take All" button functionally useless. Basically Bethesda RPGs. There’s so many types of junk item in Mass Effect Andromeda there’s not just a “take all” option on item pick up but a “sell all junk” button when you visit merchants. I don’t mind it as I’m raking it in but it hardly seems necessary. I think DA Inquisition had the same item/shop management so finding items like “leather bindings” would have made more sense back then.

I absolutely wasn't aware of this - not having played it - but apparently Far Cry 1 had hit-scan weapons, but the visible bullets had travel time. So in actuality those are apparently not the real bullets and the game induces the player to lead with the aim and essentially tricks you into playing incorrectly.

Since I’m replaying Alien Isolation and just turned the option off I want to repeat an earlier entry and say any use of the in-controller speaker is bad.
1. The sound quality is horrendous 2. I find it immediately immersion breaking 3. Sometimes if I don’t realise a game uses it it makes me jump

Controller volume always gets disabled immediately for PS... It's such an utterly shit feature, it just sounds so tinny and pathetic and as you say just breaks the immersion rather than adds to it.

This might just be a personal one for me and me alone, but I can't stand it in large semi-open-world type games where there are specific save spots rather than a "save anywhere" option in the menu or regular checkpoints. I'm just lazy, I don't want to climb the same mountain five times or breeze past the same nine guys just because the tenth keeps killing me.

I'm pretty sure it's been mentioned already, but stealth sections in games that aren't stealth games.
If a game is designed with stealth in mind from the get go it can work and be fun, but some games just shoe-horn stealth into games not really built with that in mind and it's usually abysmal.

When you can swim only in the direction you're looking. As in, your game has a jump and a crouch button, so please let me just swim up or down without having to look in that direction (especially annoying with a controller). Generally, games acting like your head and eyes are locked in the direction your body is facing. Off the top of my head, Outer Wilds comes to mind, where strafing is slow because your character is literally stepping sideways. People can look sideways while walking forward, you know. Like this:

Games not having a load game option in the in game menu so you have to quit back to the main menu first if you want to reload
Resident Evil 4 Remake retroactively updating your save file if you die/continue/reload unless you quit to the main menu and then reload your game from there
Weapons magically attaching to characters backs when they're not using them (this is something I've started noticing and being irrationally bothered by)

The weapons thing doesn't bother me, as I've played enough MMOs and similar where weapons just attach to backs/hips with no sheath or straps I'm just immune to it. But it is pretty odd when you think about it.

Straps would probably be the simplest solution.
I think the issue would be it then creates extra possibility for models clipping, especially if a game has complex and varied armour. I'd probably find that kind of artifacts more annoying than them being absent.
Plus some massive fantasy swords would not work in sheathes.
I did see something where they had a magnetic "sheathe" to keep the sword / axe on back. Maybe everyone just has that.

Let's be honest though, if bagspace wasn't a thing, and you could only carry realistic ammounts, most games wouldn't function. Or it would be a bit tedious, or everyone would like that guy from Death Stranding.

Unnatural pauses in spoken dialogue during conversations between characters whilst the camera switches from one to the other.
JRPGs are the biggest offender but it's not exclusive to that genre. Completely ruins the flow and makes immersing yourself in a game and its' characters pretty tricky.
