I remember Drakensang having a bunch of side-areas that could only be entered once you had talked to the relevant questgiver. That seems like it's good design. Not well implemented or elegant of course but it's better than stumbling around in an area that might not function without the quest.
But granted, it can be disorienting or pull you out of the experience. Especially when the rules of the world aren't yet clear.
Conversely, it can also be annoying that a game let's the player go everywhere in the open world, but puts high-level enemies everywhere that cannot be properly interacted with. Like in Cyberpunk for instance. It's especially annoying when there is no reason for why certain enemies would be so much more powerful than other similar enemies in other parts of the map.

non functional doors painted onto walls like wile-e-coyote got busy. sometimes they'll make a pretense of saying "it's locked", well is it locked to my axe, motherfucker? the axe that in the last village cleaved the skull of a motherfuckin dragon? yeah, four planks of "locked" wood gonna be all impervious is it? paint a door on a wall and that door damn well better open one way or another THIS! The better games look the more this type of thing totally breaks immersion for me.

Another thing that Skyrim did right. If there was a door, there was something behind it. You could count the exceptions with the fingers on one hand.

Metroidvanias not showing you an automap in a new area until you find/buy a map somewhere, like in Hollow Knight. It was a terrible idea in HK already, please everyone don't ape that shit and just give me an automap!
Similarly with map markers having to be bought or found or whatever. What's wrong with just giving you them from the start?
And seriously, generally stop copying HK's and Dark Souls' shitty ideas everywhere! Damn these two games and their hateful influence. If I ever make a game, my advertising will say "No stupid Dark Souls/Hollow Knight mechanics".

I wouldn't say that I hate it when that's not the case, but it's always lovely that when a quest gets generated it then will always exist in the world completely irregardless of the question if I do or do not have it in my quest log. Same for characters recognizing that a quest is already done when I speak to them the first time. Just nice conveniences that make a game world feel more convincing.

Not so much a mechanic as a personal annoyance, but voice actors doing literally nothing to differentiate one of their characters from another from one game to the next, or even in the same game, bugs me. There are some who genuinely surprise me and I never would have imagined they had such a vocal range, but there are others who just record every single one without variation.
I've heard Brandon Keeners' voice as an absolute shitload of Fallout 4 characters, and then he turns up in the Far Harbour and Nuka World DLCs as other new characters as well, and literally every single one of them, I'm sitting there thinking "You are Garrus Vakarian."
It's even stranger when you hear a voice like that in games before you ever see them in anything live action, pretty much turns me into that meme of diCaprio pointing at the screen in recognition.
Same thing when I was replaying Watch Dogs: Legion last night. Every time Sabine spoke, my mind couldn't stop going "James Kidd/Mary Read from Black Flag".
Star Trek: Resurgence was another recent one, I kept thinking Lieutenant Itasca was actually Evie Frye from Ass Creed: Syndicate wearing alien makeup.

Good point. One reason why I didn't care for the voice acting in Pillars 2 was simply that most characters and their voices were not memorable. So in the end the important characters still stood out and most of the voice acting just felt unnecessary or worse - annoying.

Yeah, there was the time where it felt like Nolan North voiced about 90% of male videogame protagonists.
It became such a meme that one of the Saints Row games had his voice option as just 'Nolan North'.

Ironically, I'm playing Uncharted 4 at the moment and I can't for the life of me tell who's speaking, Nate or Sam, when they're together.
So much so, I thought North must have voiced both, but nope.

Yeah, Troy Baker has long taken Nolan North’s spot as the go-to voice actor for MALE PROTAGONIST. Just as Laura Bailey has taken the female lead spot from Jennifer Hale.

I guess part of the issue is that Nate is often hanging off something and Sam is usually out of shot, which doesn't help not being able to see who's lips are moving, which is a huge human conversational thing.
