The De Menezes killing should have undoubtedly lead to her resignation. Even putting aside all the incompetence and procedural failings in that case, the fact that they deliberately allowed the false media narrative that he had vaulted a barrier to persist was utterly shameful.

Given that, in my lifetime, I think this is the only case like this that I've heard of, you'd have hoped that the police response could have been along the lines of "We know that this has been a shocking event, but the public can be assured that this was a freak isolated incident and they can still put trust in our officers".
That they didn't say that is kinda worrying.
